Health Requirements
It is very important that students complete all the required health requirement as determined by the
Illinois State Board of Education and the Illinois Department of Public Health.
All required physical and immunization forms are due on the first day of school.
As per DU Policy, students who do not meet physical exam and immunization requirements are subject to exclusion from school.
Exclusion will not occur without prior notice.
Check with your child’s doctor or public health clinic. However, doctor’s offices and clinics may only keep immunization records for a few years. Check with your state’s health department.

*1 : Current IL Physical Examination (within 1 year of the first day of school) including all required immunizations and must be completed by appropriate medical personnel. Out of state/country records must be on an Illinois health form or comparable form and dated within one year of the first day of school. |
*2 : Current IL Physical Examination (within 1 year of first day of school) including all required immunizations and must be completed by appropriate medical personnel. Out of state/country health records must be on an Illinois health form and dated within one year of the first day of school. Physical exam forms are due by August 11. Physicals must also show proof of students receiving one dose of Tdap (defined as tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last DTap, DT, Td dose. In addition, they must also show proof of one dose of Meningococcal Meningitis vaccines given on or after their 11th birthday, 2 Varicella, 2 MMR vaccines and 3 Hepatitis Vaccines |
*3 : All ninth graders are required to have a complete IL physical examination dated within one year of entrance into grade nine. All immunizations must also be up-to-date. All 9th grade students are now required to have 2 chicken pox vaccines. Please check with your doctor. |
