Below are items/documents and health requirement for the 2019-2020. It is very important that students complete all the required health requirement as determined by the Illinois State Board of Education and the Illinois Department of Public Health. All required physical and immunization forms are due no later than August 16, 2019. As per DU Policy, students who do not meet physical exam and immunization requirements by August 23, 2019 are subject to exclusion from school. Exclusion will not occur without prior notice. ALL STUDENTS - Copy of Birth Certificate - School Supplies Preschool - Physical Exam including all required immunizations. KG - Physical Exam including all required immunizations. - New Dental Exam. - Eye/Vision Examination.

2nd Grade - New Dental Exam. 6th Grade - Physical Exam including all required immunizations. - New Dental Exam. 9th Grade - Physical Exam including all required immunizations. Any student entering Illinois Schools for the first time - Physical Exam including all required immunizations. - Eye/Vision Examination.
