Preschool Parents

The following examinations are required for entrance into preschool:

  1. Current IL Physical Examination (within 1 year of the first day of school) including all required immunizations and must be completed by appropriate medical personnel. Out of state/country records must be on an Illinois health form or comparable form and dated within one year of the first day of school. Physical exam forms are due by August 11.
  2. Copy of Certified Birth Certificate

Kindergarten Parents

The following examinations are required for entrance into kindergarten:

  1. Current IL Physical Examination (dated within 1 year of the first day of school) including all required immunizations and must be completed by appropriate medical personnel. Out of state/country records must be on an Illinois health form and dated within one year of the first day of school. Physical exam forms are due by August 11.
  2. IL Dental Examination – This exam is to be submitted by August 11. (dated on or after 11/11/2016)
  3. IL Vision Examination – This exam is to be submitted by August 11. (dated on or after 8/11/2016)

Second Grade Parents

An IL dental examination is required for all students by August 11. (dated on or after 11/11/2016)

Sixth Grade parents

The following examinations are required for entrance into sixth grade:

  1. Current IL Physical Examination (within 1 year of first day of school) including all required immunizations and must be completed by appropriate medical personnel. Out of state/country health records must be on an Illinois health form and dated within one year of the first day of school. Physical exam forms are due by August 11. Physicals must also show proof of students receiving one dose of Tdap (defined as tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last DTap, DT, Td dose. In addition, they must also show proof of one dose of Meningococcal Meningitis vaccines given on or after their 11th birthday, 2 Varicella, 2 MMR vaccines and 3 Hepatitis Vaccines.
  2. IL Dental Examination A dental examination is required for all students by August 11. (dated on or after 11/11/2016)

Ninth Grade Parents

All ninth graders are required to have a complete IL physical examination dated within one year of entrance into grade nine. All immunizations must also be up-to-date. All 9th grade students are now required to have 2 chicken pox vaccines. Please check with your doctor.

Any student entering Illinois Schools for the first time

The following examinations are required for entrance into school:

  1. IL Physical Examination including all required immunizations must be completed by appropriate medical personnel. Out of the state/country health records must be on an Illinois Health form dated within one year of the first day of school.
  2. IL Dental Examination
  3. IL Vision Examination

No child can start the school year without a completed physical examination from the appropriate medical personnel. Students who fail to submit proof of the required immunizations or health examination by October 15th (or the first day of enrollment if the student is enrolling after October 15th), shall be barred from attending school. A student who does not have proper proof of a health examination or immunizations will receive a “Notice of Exclusion from School” signed by the principal excluding the student.
